Harbourmaster Navigational Incident InvestigationFollowing a navigational incident on river or estuary the Area Harbourmaster as part of the investigation process into the incident will request a written report from persons involved in the incident. The following form Incident Report Form (Authorised Pilot / PEC Holder / Master) - Form 221 - should be completed and returned to the Area Harbour Master.
